Payment/Insurance

I am in network with the the following insurances:​

If your insurance provider is not listed, I can provide you with a receipt to submit to your insurance company for possible reimbursement. 

 

If your insurance plan covers nutrition counseling and allows you to see out-of-network providers, part or all of our visits may be covered.  It is your responsibility to contact your insurance company to clarify what your plan’s benefits allow. 

 

Full payment is due at the time of service.
